SUMMARY OF POSITION:
MSW Clinic Social Workers are vital members of a dynamic team at Penn State Hershey Medical Center. As a member of the multi-disciplinary team you will provide psychosocial support to patients and families through the continuum of care from admission to discharge. MSW Inpatient Clinical Social Workers actively participates in creating and implementing discharge plans, provide patient and family advocacy, assist patients who require post-acute care, deliver emotional support to patients and families in crisis and/or managing complex medical conditions, provide bereavement and end of life counseling, assist patients with access to care, and resources to address social determinants of health. Penn State Hershey Medical Center promotes individual professional growth and development through certification, our social work professional clinical ladder program, case management continuing education, and participation on department/hospital committees. MINIMUM QUALIFICATIONS:
Masters of Social Work degree from a Council of Social Work Education school of social workÂ
KNOWLEDGE, SKILLS, & ABILITIES:
Ability to establish and maintain a collaborative relationship with internal and external health professionals
Excellent written and oral communication skills
Ability to use problem solving techniques to achieve desired results and effective change
Ability to function effectively in a dynamic and rapidly changing environment
Ability to organize, prioritize, and time manage responsibilities
Basic computer skills with effective keyboarding abilities
PREFERRED QUALIFICATIONS:
Minimum of two years of acute care case management/social work experience
Knowledge of PC based computer software (Cerner, Navihealth/Curaspan)
Current social work licensure in Pennsylvania
Certification in case management from a nationally recognized organization
