Overview
Atlantic Health System is seeking a full- time Social Worker for Case Management Department to work 8 hour shifts between the hours of 8a-6p and 1 Sat. per month as needed.
Responsibilities
Atlantic Health System is seeking a full time Social Worker at Overlook Medical Center.
- Social Worker provides psychosocial assessments, counseling and facilitation of discharge plan in all areas within the hospital. Responsibilities include:
- Conducting psychosocial assessments
- Discharge planning for post acute needs
- Obtaining insurance authorizations
- Making referrals to appropriate community resources
- Active participation in family and team meetings
- Providing supportive counseling
- Crisis intervention
- Assessing and making referrals for abuse and neglect (adult and pediatric), providing support and counseling at the end of life
Qualifications
Qualifications:
Minimum Education:
- MSW required
License/Certifications:
- Current LSW or LCSW license in the State of New Jersey
Relevant Experience:
- 1 year previous hospital SW experience and/or acute care experience preferred (knowledge of systems - financial, insurance, advocacy, legal, knowledge of community resources)
- Familiarity with Epic preferred
