Overview
Atlantic Health System is seeking a full- time Social Worker for Case Management Department to work 8 hour shifts between the hours of 8a-6p and 1 Sat. per month as needed.
Responsibilities
Atlantic Health System is seeking a full time Social Worker at Overlook Medical Center.
Social Worker provides psychosocial assessments, counseling and facilitation of discharge plan in all areas within the hospital. Responsibilities include:
Conducting psychosocial assessments
Discharge planning for post acute needs
Obtaining insurance authorizations
Making referrals to appropriate community resources
Active participation in family and team meetings
Providing supportive counseling
Crisis intervention
Assessing and making referrals for abuse and neglect (adult and pediatric), providing support and counseling at the end of life
Qualifications
Qualifications:
Minimum Education:
MSW required
License/Certifications:
Current LSW or LCSW license in the State of New Jersey
Relevant Experience:
1 year previous hospital SW experience and/or acute care experience preferred (knowledge of systems - financial, insurance, advocacy, legal, knowledge of community resources)
Familiarity with Epic preferred
