Atlantic Health System is seeking a per diem Social Work for the Care Management Department at Morristown Medical Center to work 8am-4pm including one weekend shift per month.
Responsibilities
Essential Functions/Responsibilities:
Responsibilities will include discharge planning for post-acute care needs.
Insurance authorizations for sub-acute and rehab facilities.
Conducting psychosocial assessments, family meeting and acting as an integral part of reducing LOS.
Counsels, assesses, and assists individuals of all ages and families or groups who are affected by illnesses or injuries, including rape, physical abuse, cancer, AIDS, and Alzheimer's, that may impair their ability to function normally.
Provides psychosocial assessments to determine patient needs, makes appropriate referrals, provides counseling and supportive services to patients/families, makes referrals to the Division of Child Protection and Permanency (DCP&P)
Identifies and maintains funding sources which supports the Family Counseling & Guidance Program.
Maintains statistical information and analysis of data, develops outcome and measures for program evaluation, program development and facilitation of program.

Qualifications
Minimum Education:
MSW from an accredited school of Social Work.
LSW
Minimum Experience:
Previous adult medical hospital social work experience including discharge planning required.
Familiarity with Epic desired.
License/Certification:
NJ Social Work License (LSW or LCSW) required.
