Incumbent serves as the Health Behavior Program Coordinator at the GS-12 grade. The Social Worker provides direct patient care, teaching/training services, and program evaluation/outcomes services. Incumbent is administratively responsible for the program and effectively develops and implements health behavior interventions for the promotion of general health and to address health risk behaviors as part of disease prevention and chronic disease management Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
