Treating over 330,000 patients every year, the University of Maryland Medical Center is at the vital core of Maryland's health care system and community. The R Adams Cowley Shock Trauma Center, the world's first center dedicated to trauma, treats more than 7,500 critically injured patients a year with an incredible 97 percent survival rate. We also have one of the nation's largest kidney and pancreas transplant programs at The Joseph and Corinne Schwartz Division of Transplantation, home to Maryland's first steroid-free protocol and pancreas/kidney transplant. From our National Cancer Institute-designated UM Marlene and Stewart Greenebaum Cancer Center to The University of Maryland's Children's Hospital, one of the largest pediatric centers in the state, we are dedicated to saving and transforming lives.

The Coordinated Care Center (C3 Clinic) provides individualized intensive, comprehensive, coordinated ambulatory care for vulnerable patients. Our vision is to holistically address our patients' needs to improve health and well-being through coordinated medical services with emphasis on social determinants affecting health outcomes. The center offers a patient-centered, evidence-based multidisciplinary approach with a team that provides care coordination to post-hospital discharges for at least 30 days. Patients are ultimately connected to community primary care providers or comprehensive specialty care clinics for continued long-term management of their conditions.

Some things our multidisciplinary team assists patients with are:

• Appropriate and affordable medical care (FQHC, immigrant services, primary & secondary care, utilizing our high risk model bridge clinic (the Coordinated Care Center) for those who are medically complex and require services that are of a higher intensity, longer duration or higher frequency visits than can be met in the community)
• Social programs (food resources, supplemental income)
• Services to access care (ID, insurance)
• Transportation (mobility, medical assistance transportation, bus services)
• Housing (shelters, affordable housing resources, assisted living)
• Employment resources
• Treatment for substance use disorder
• Mental health services
