The VA Northeast Ohio Healthcare System is actively recruiting for a Social Worker within the Behavioral Health Interdisciplinary Program (BHIP). The incumbent in this position functions as a Care Coordinator for Veterans on a BHIP team in the ambulatory mental health clinics at VANEOHS and is responsible for implementation of the Collaborative Chronic Care Model to facilitate coordinated, proactive, population-based care and continuity of care for Veterans receiving mental health services.
