Primary Care Mental Health Integration (PCMHI) functions as an extension of the PACT Team and serves as the access point to mental health. The Incumbent works within the PCMHI Model serving as a Senior Social Worker/Behavioral Health Consult. He/she works closely with the physician team lead, nurse case manager and all teamlets to provide brief assessment and consultation/referral for veterans with psychosocial concerns covering the full spectrum of disorders. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
