Job DescriptionIn the setting of the Comprehensive CARE Center, a multidisciplinary transitional clinic the Clinical Social Worker is responsible for completing intake assessments, assisting with treatment planning, monitoring patient progress, facilitating patient and family meetings, and coordinating and implementing discharge services for patients. Independently practices social work and receives limited consultation, functioning as a member of the multidisciplinary healthcare team to provide case management, therapeutic crisis intervention, biopsychosocial assessment, clinical evaluation, therapy, counseling, referral, treatment planning, and other clinical social work services within their practice area.
