Job DescriptionPACT (Program for Assertive Community Treatment) is an evidence-based practice that improves outcomes for people with chronic and severe mental illness who are most at-risk of psychiatric crisis and hospitalization and involvement in the criminal justice system.PACT is a multidisciplinary team approach with assertive outreach in the community. The goal is to reduce hospitalization, increase housing stability and improve quality of life for clients.
