OverviewThe Behavioral Health Complex Case Manager is responsible for assisting patients and families in managing mental health, substance use and psychosocial issues by developing disposition and execution of discharge plan, with a focus on long length of stay and complex patients. Serves as a patient/family advocate and a communication link between the physician, patient, family, health care team, community resources, and payers to reduce length of stay and recidivism. Anticipates potential delays in the disposition and acts pro-actively to avoid these delays. Functions as a resource and placement specialist for the team with strong working knowledge of MH/SU levels of care.
