Mental Health Therapist
The Mental Health Therapist provides clinical mental health services to assigned patients in the Adolescent Partial Hospitalization Program (Adolescent PHP), including Group, Individual, and Family Therapy, Admission and Discharge Assessments, Case Management, and Discharge Planning. The Mental Health Therapist also acts as a liaison between the Adolescent PHP and community providers and between the patient, family, and the Adolescent PHP team and assists other members of the treatment team in coordinating and implementing the patients' individualized treatment plans. RESPONSIBILITIES Completes psychosocial on assigned patients and assists treatment team in developing each patient's individualized treatment plan.
