Job Summary: The Network Senior Social Worker will provide medical or mental health related supportive services to patients at Westchester Medical Center Health Network. The Network Senior Social Worker role is primarily based in the outpatient setting, including the Center for Women's Health Equity and APS Practice sites with some minor inpatient service to ensure continuity of care across clinical settings. The candidate will be required to perform complex care coordination within the WMC Health Network but will be based out of the main campus in Valhalla, NY.
