We are seeking a Social Worker to join the Palliative Care team. The role of the PACT Social Worker is to support PACT inpatients and outpatients in partnership with the interdisciplinary palliative care team. The PACT social worker partners with the primary disease / unit based social worker and psychosocial providers to facilitate family communication with the medical team. The PACT social worker joins the Palliative Care IDT to engage in family decision-making and goals of care discussions, assist with developing an end of life care plan as appropriate, initiate / support hospice referrals and discharge coordination, and support transition to bereavement when indicated.
