Job Description Do you have a passion for healthcare and a heart of service? Look no further than Saint Luke's!Responsible and accountable for assessment of psychological, social, emotional, cultural and financial needs, and the resulting treatment that impacts patients' health and recovery. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. Participates in program planning and development to meet the needs of customers within the entity, health system and community.
