The Social Worker in this position will report to the Health Care for Homeless Veterans Programs Supervisor, in the Social Work Executive Service Line at the Fargo VA Health Care System. They are responsible for outreach services to homeless veterans in communities and rural areas outside the main Medical Center campus. This work typically requires extensive coordination of services with a wide range of veterans and providers in and outside of the Medical Center and the VA in general. The Social Worker makes assessment of and treatment plans for homeless veterans. Specific duties are as follows: Plans, develops and maintains a network of relationships with community resources and other advocates for the homeless to effectively screen and identify veteran members of the homeless population
