DescriptionThis candidate will provide care coordination for patients and families served by the Perlman Center, a specialty center, caring for patients with cerebral palsy and other neuromuscular disorders. Plays an active role in the Perlman multi-disciplinary care team, insuring quality care and providing medical and social resource coordination within CCHMC as well as various community systems. This candidate will work on a multidisciplinary team in the cerebral palsy clinic assessing the psychosocial needs of patients from birth through adulthood as well as the challenges that families have when caring for children with special needs.There are opportunities to participate in newly developed and exciting initiatives within the quickly growing CP p
