This position is a Geriatrics and Extended Care Social Worker at the Community Living Centers. The incumbent is primarily responsible for completing psychosocial history and assessments, discharge planning, completing psychosocial portions of care plans and the Minimum Data Set. The incumbent facilitates Team meetings, emphasizing culture transformation, including resident centered care planning. Provides coverage in other areas as needed. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
