SUMMARY: Functioning under the Case Management department, the Emergency Department (ED) Social Worker serves on an interdisciplinary team addressing immediate needs for ED patients. The ED Social Worker completes psychosocial assessments, provides trauma support and crisis counseling, coordinates community resource referrals, collaborates with other providers to reduce readmissions, and coordinates effective and efficient discharge planning. EXPERIENCE: Minimum one year in the healthcare/medical social work field (i.e., hospital, home health or nursing home). SPECIAL SKILLS: Excellent communication and interpersonal skills; trauma informed care, excellent assessment skills. OTHER: Computer skills; schedule flexibility. LICENSE/CERT: Required: LMSW (Licensed Master Social Worker) EDUCATION: Required: Masters - Social Work (CSWE approved)
