Social Worker-Outpatient PACT/ GM & Specialty Clinics
The primary responsibilities of this position are to provide social work services to PACT/GM & Specialty clinics within VA Ann Arbor Ambulatory Care Services. Primarily responsible for providing Outpatient Social Work services to Veterans who are receiving medical care predominately in the Patient Aligned Care Team (PACT) clinics at the Toledo CBOC, Specialty clinics, Emergency Department and as a part of SW triage walk-in clinic located within the hospital setting. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
