The Social Worker provides outreach, intake and/or case management services to Homeless Veterans and is responsible for outreach and/or case management services to homeless Veterans in the community, in shelters, Veteran's homes or anywhere homeless Veterans may be located. Assignments may involve providing continuing social work services at a field location alone or with subordinate social work staff members without a supervisor available for immediate consultation. The Health Care for Homeless Veterans (HCHV) Social Worker: Makes professional decisions and recommendations with infrequent recourse to supervisory guidance. Plans, develops and maintains a network of relationships with community resources and other advocates for the homeless to effectively screen and identify Veteran member
