Principle Accountabilities:
- Evaluates patients and establishes a plan of care. Uses current standards of practice. Determines patient functional goals and incorporates them into time-oriented, measurable team goals. Documents evaluations within established time frames. Uses age-appropriate language and evaluation tools and techniques to establish a problem list.
- Treats patients according to a plan of care. Utilizes evidence-based body of knowledge, current standards of practice and techniques. Provides goal-oriented treatment based on predetermined patient needs. Manages a full case load in a timely manner meeting acceptable productivity standards. Reassesses and adjusts the patient's care plan based on changes in patient's status.
- Engages family/caregivers in patient's plan of care and discharge planning. Utilizes age-appropriate techniques to treat patients. Considers cultural & religious preferences in developing plan of care.
- Documents all patient care. Records all treatments, re-evaluations and progress toward goals and updates functional goals as needed. Writes discharge summaries, letters, and reports in a complete and professional manner. Completes outcome and PI measures as needed. Enters all billing data within established time frames.
- Interfaces with other professionals and students. Coordinates with the team and patient/family to design a home program. Participates in patient care rounds and makes recommendations to other team members as needed to implement the interdisciplinary plan of care. Coordinates with outside agencies for equipment, placement and services, acts as a preceptor to new staff members and serves as a mentor to rotating staff.
- Participates in the overall day-to-day operations of the department. Acts to conserve all resources of the department. Evaluates need for equipment and supplies and communicates to the appropriate person. Monitors and responds appropriately to new referrals, schedules new patients within established time frames. Collaborates with staff members at all levels to provide coordination of care, responds to changes in schedule and is proactive in meeting staffing needs of department and is flexible in meeting customers' needs.
- Maintains a safe environment for patients and staff members. Adheres to all safety standards to assure a safe rehabilitation environment for the patients. Demonstrates knowledge of safety standards and practices. Evaluates safety of all clinical equipment and assures its optimal performance. Recognizes emergency situations and responds appropriately.
- Educates patients and families and caregivers. Uses age-appropriate teaching methods and tools to educate patients and families. Uses culturally sensitive language, teaching materials and techniques in educating patients and families. Evaluates effectiveness of teaching and revises teaching plan in collaboration with patients and caregivers.
- Responsible for own competency and clinical expertise.
- Responsible for own practice standards and clinical performance.
Master's degree in speech-language pathology from an accredited graduate program;
NJ State License or eligible for same (NJ Temp license, for completion of CCC)
